Plantar Fasciitis

Plantar fasciitis is the leading cause of heel pain in adults, affecting millions of Americans each year. The plantar fascia, which is a thick band of tissue running along the bottom of your foot, becomes inflamed, causing sharp, stabbing pain especially with the first steps of the morning. The right footwear provides the arch support and cushioning needed to reduce tension on the fascia and allow healing.

Symptoms

Signs You May Have Plantar Fasciitis

Sharp heel pain with first steps in the morning

Pain after long periods of rest

Stiffness or tenderness along the bottom of the foot

Pain that worsens after exercise

Swelling around the heel area

Footwear Guidance

What to Look for in a Shoe

Evidence-based footwear criteria specific to this condition.

Heel Drop

Recommended: 8-12mm

Moderate heel drop reduces tension on the plantar fascia by shortening the functional length the fascia must span during the push-off phase of gait. Research published in the Journal of Foot and Ankle Research supports elevated heel geometry as a conservative management strategy for fascial overload.

Avoid below 4mm

Cushioning

maximum

Maximum cushioning attenuates ground reaction forces transmitted to the heel and midfoot, reducing repetitive stress on the plantar fascia insertion point at the calcaneus.

Avoid: minimal, barefoot-style, zero-drop

Stability vs. Neutral

stability

Overpronation increases medial fascial tension with every footstrike. Stability shoes with medial post or guide rail technology resist excessive inward roll, reducing cumulative fascial load across a day of walking or running.

Exception: Neutral runners with confirmed high arches and supination may perform better in neutral-cushioned shoes. Gait analysis by a podiatrist or certified pedorthist is recommended when arch type is uncertain.

Width

Recommended: D, 2E, 4E

Standard to wide fit prevents forefoot compression that can alter weight distribution and increase compensatory fascial loading. A proper fit across the entire foot reduces gait compensations.

Insoles and Orthotics

Removable footbeds are strongly preferred. Custom orthotics prescribed by a podiatrist are one of the most evidence-supported conservative treatments for plantar fasciitis and require a shoe with adequate interior depth to accommodate them.

Understanding This Condition

What Is Happening in Your Foot

The plantar fascia is a thick band of connective tissue that runs along the bottom of the foot, connecting the heel bone to the base of the toes. It acts as a passive tension structure that supports the arch during every step. When this tissue is repeatedly overstressed - through prolonged standing, sudden activity increases, or poor footwear support - micro-tears accumulate at the heel attachment point, causing inflammation and pain.

Biomechanics

During normal gait, the plantar fascia undergoes a windlass mechanism - as the toes dorsiflex at push-off, the fascia tightens and raises the arch, contributing to propulsive force. When the fascia is already inflamed, this mechanism becomes painful. Footwear that limits excessive toe extension or provides a moderate heel raise reduces fascial tension during this critical phase of the gait cycle.

Why Symptoms Behave the Way They Do

During sleep, the foot rests in a plantarflexed (toes-pointed) position, allowing the plantar fascia to contract and shorten overnight. The first steps of the morning abruptly stretch this shortened tissue, causing the characteristic sharp start-up pain. Night splints and supportive footwear worn immediately upon rising address this mechanism directly.

What Makes It Worse

Walking barefoot on hard floors, especially first thing in the morning

Sudden increases in walking or running mileage

Prolonged standing on concrete or tile without cushioned footwear

Wearing flat, unsupportive shoes or sandals

Tight calf muscles and Achilles tendon, which increase fascial tension

Obesity, which increases mechanical load on the fascia with every step

What Helps

Supportive footwear with arch support and deep heel cup worn throughout the day

Plantar fascia and calf stretching before first steps in the morning

Ice application to the heel after prolonged activity

Custom or over-the-counter orthotics that distribute pressure away from the fascia

Gradual return to activity with mileage increases no greater than 10 percent per week

Common Misconceptions

Myth: Complete rest is the best treatment for plantar fasciitis.

Research suggests that total inactivity allows the plantar fascia to tighten and can slow recovery. Gentle, low-impact movement in supportive footwear combined with stretching is more effective than immobilization for most patients.

Myth: Plantar fasciitis is caused by a heel spur.

Heel spurs are calcium deposits that frequently appear on imaging alongside plantar fasciitis, but research indicates they are a consequence of chronic fascial tension rather than a cause of pain. Many people with large heel spurs have no symptoms at all.

Myth: Once you have plantar fasciitis it never fully heals.

Research suggests that approximately 90 percent of plantar fasciitis cases resolve within 12 months with appropriate conservative treatment including proper footwear, stretching, and activity modification.

Who This Affects

Plantar Fasciitis by Occupation

How this condition presents differently depending on how you spend your day.

nurses

Nurses and healthcare workers are among the highest-risk groups for plantar fasciitis.

Twelve-hour shifts on hard hospital floors with limited opportunity to sit create sustained fascial loading throughout the day. Many hospital dress codes restrict footwear choices, narrowing options. Shift-to-shift schedule variation means the foot rarely has consistent recovery time.

Shoe Priority

Maximum cushioning with slip-resistant outsole, removable footbed for orthotics, and a heel drop of 8mm or higher.

Read the nurses guide

teachers

Teachers average 7,000 or more steps per day on hard classroom and corridor floors.

Concrete and tile floors with no anti-fatigue matting are standard in most school environments. Teachers rarely have scheduled sit-down breaks during the school day. Back-to-school season often triggers plantar fasciitis flares as activity levels increase sharply after summer.

Shoe Priority

Arch support with all-day comfort, professional appearance compatible with school environments, and a cushioned midsole rated for full-day standing.

Read the teachers guide

standing workers

Retail, warehouse, and factory workers face sustained fascial stress across every shift.

Anti-fatigue mats help but do not eliminate fascial loading. Shift workers often change shoes between work and home without maintaining arch support consistency. Hard concrete floors in warehouse and retail environments are among the highest-risk surfaces for plantar fasciitis development.

Shoe Priority

Wide fit with deep heel cup, high arch support, and a midsole rated for 8 or more hours of continuous standing.

Read the standing workers guide

runners

Runners are the single most common demographic for plantar fasciitis onset.

Mileage increases, inadequate recovery footwear between training sessions, and transitioning to lower heel drop shoes too quickly are the primary triggers. Many runners continue training through early plantar fasciitis symptoms, converting an acute case into a chronic one.

Shoe Priority

High arch support in both training and recovery footwear, heel drop of 8mm or higher, and a structured midsole that does not compress excessively at speed.

Read the runners guide

seniors

Adults over 60 face elevated plantar fasciitis risk due to reduced tissue elasticity and increased time spent in unsupportive footwear at home.

Many older adults wear flat slippers or go barefoot indoors, which places unsupported stress on the fascia throughout the day. Reduced calf flexibility with age increases fascial tension. Balance considerations mean seniors also need stability, creating a dual requirement.

Shoe Priority

Stability with arch support, slip-resistant outsole, easy entry design, and a cushioned heel that does not sacrifice ground feel needed for balance.

Read the seniors guide

restaurant workers

Restaurant and kitchen workers stand on hard tile floors for shifts of 8 to 12 hours, often in footwear chosen for slip resistance alone.

Slip-resistant footwear is mandatory in most kitchens but is often flat and poorly cushioned. Workers may stand in place for extended periods during prep work, creating sustained static loading on the fascia rather than the rhythmic loading of walking.

Shoe Priority

Slip-resistant outsole combined with arch support and heel cushioning - these requirements are not mutually exclusive and several APMA-accepted shoes meet both criteria.

Read the restaurant workers guide

walkers

Fitness walkers and people building walking habits from low base fitness are at elevated risk for plantar fasciitis onset.

Walking volume increases too quickly as new walkers build habits, particularly common after medical advice to increase daily steps. Worn-down walking shoes with collapsed midsoles are a frequent contributing factor that goes unnoticed.

Shoe Priority

Supportive walking shoe with arch support, adequate heel cushioning, and a midsole that has not been compressed beyond its useful life - most walking shoes should be replaced every 300 to 500 miles.
